A treatment plan is any written outline of therapy developed by a mental health professional for a particular patient. When therapists develop a treatment plan with a patients, they always identify the problem or issue to be treated, the goals and methods of therapy, and an estimated time schedule. Usually the patient is involved in making up the plan, and sometimes it may be developed by a team of professionals. The plan is always open to changes, and usually includes periodic progress reports.

For example, when a patient enters a substance abuse treatment center, he will undergo a series of mental and physical examinations and tests, often on the first day. Afterwards, his case manager, psychiatrist, psychologist and other members of his therapeutic team will sit down with him and write out a treatment plan. It may include chemical detoxification, individual and group therapy, art, drama and music therapy, sports and yoga, classes in relaxation and nutrition, 12-step meetings, and involvement with his family in his program. The final part of the treatment plan will include aftercare support and therapy once he returns home.
